How an ERP-Based IOP Differs From Weekly OCD Treatment

Weekly sessions work well for many people, especially those with mild to moderate symptoms. But when OCD becomes severe, progress can stall, not because the treatment is wrong, but because the volume and intensity of symptoms require more frequent, structured ERP intervention.

 

In an Intensive Outpatient Program:

  • You meet several hours per week, giving you the repetition needed to retrain your brain faster.

  • ERP work is deeper and more immersive, allowing us to tackle complex rituals, mental compulsions, avoidance patterns, and fear cycles.

  • There’s less time for compulsions to rebuild, which accelerates progress.

  • You get a clearer structure and accountability, which is crucial when OCD feels overwhelming or when avoidance takes over.

An OCD IOP isn’t just “more therapy.” It’s a different level of intensity, built for people who need more support, more structure, and more frequent ERP practice to move forward.

Who the OCD IOP Is For?

This intensive program is appropriate for teens and adults anywhere in Colorado who are:

  • Experiencing moderate to severe OCD

  • Spending over two hours per day on obsessions or compulsions

  • Avoiding major activities such as work, school, driving, self-care, or relationships

  • Struggling to follow through with ERP due to fear, avoidance, or exhaustion

  • Feeling stuck, despite traditional weekly ERP sessions

  • Motivated to make change, but overwhelmed by how intense OCD feels

  • Dealing with more complex ritual cycles, such as mental review, rumination, or reassurance loops

  • Experiencing a recent flare-up, relapse, or major increase in symptoms

The IOP is also a strong fit if you’ve recently come out of a higher-level program (PHP/RTC/Rogers-type programs) and need continued structured ERP without dropping abruptly to once-weekly care.

Frequency and Duration

  • 4-9 hours per week, depending on symptom severity and goals

  • Typically 2–4 weeks, but flexible based on progress

  • Sessions scheduled in 2 hour or 3 hour blocks

Core Components

  • Individual ERP sessions several times per week

  • Exposure planning, including in-session exposures

  • Response Prevention coaching

  • Between-session ERP assignments tailored to your daily life

  • Psychoeducation about OCD and the fear cycle

  • Optional: Parent or partner involvement for clients needing reduction in reassurance or accommodation

This means your treatment plan isn’t a one-size-fits-all schedule, it’s structured around what will be most effective for you. The program typically ranges from 4 to 9 hours per week, with sessions often stacked in several-hour blocks, 2 to 4 days per week. This format allows for the depth, repetition, and momentum needed to create meaningful change, especially when OCD has become severe or entrenched. While the program is delivered virtually to ensure access across all of Colorado, I am available for in-person sessions for those in the Denver Metro Area. It’s also common for people with more severe or entrenched symptoms to need a higher level of support than traditional once-weekly ERP sessions can provide.
